Premier Visiting Nurse — RN Case Manager

Position Description: Coordinates the overall interdisciplinary plan of care for a patient from admission to discharge, in a home care environment. Responsible for furnishing services in accordance with agency policy and procedures. Acts as the liaison between patient/family and home care personnel to ensure necessary care is provided promptly and effectively.

Currently Full Time and Per Diem positions available in Wayne, Oakland, Macomb, Washtenaw, and Flint areas.


  • Current State of Michigan RN License
  • Valid Drivers License, Auto Insurance and reliable transportation
  • Current CPR Certificate
  • Must have a pleasant disposition, a cheerful attitude, and a sincere desire to participate in aiding and supporting the needs of our patients
  • Must have patience, compassion and understanding for all patients


  • Provide care to patients in accordance with a medically approved Plan of Care
  • Perform supervisory visits for home care aide services and LPN visits as necessary
  • Skilled nursing tasks including, but not limited to, lab and specimen collection, head to toe assessment, patient/caregiver education and training, port access, start IV’s, wound care and assessment, PICC care, infusion, and G-tube care
  • Case coordination of new clients with additional therapies (i.e. PT, OT, ST, MSW)
  • Responsible for maintaining and safeguarding the medical records of our clients and maintaining confidentiality of patients’ rights
  • Admission 485’s and OASIS
  • Planning and completing duties with minimal direction
  • Using tactful, diplomatic communication techniques in all situations
  • Following up with appropriate staff, residents, or other individuals regarding reported complaints, problems, or concerns
  • Presenting a positive image for the facility by presenting a professional, sincere and kind attitude towards guests, clients and other visitors
  • Maintaining all certifications required for employment
  • On-Call during the week and weekends to alternate with other nurses
  • Attending and participating in all staff meetings
  • Participating in in-services, and with projects or committees as assigned

Physical Demands

  • Must be able to lift/push up to 50 pounds
  • Must be able to walk four miles daily
  • Must be able to lift, carry, stand, bend, squat, crawl and reach
  • Must possess sight/hearing sense, or use prosthetics that will enable these senses in order to meet the requirements of this position and the needs of our clients
  • Must be in good general health and demonstrate emotional stability in order to cope with the physical, mental and emotional stress of the position.
