The decision to address the need for home health care services is often a confusing one to make. Many aspects of our services and procedures may be new to you. Knowing what questions to ask to find an organization committed to yours or your loved one’s care may seem daunting.
In fact, each patient is provided a ‘Patient Care Advocate’ to answer non-medical questions, locate resources, and meet the needs of the patient and/or their family. In addition, each patient is provided a ‘Patient Navigator’, a nurse who navigates and/or coordinates patient appointments, provides educational materials plus provides support for pain and wound care and other medical concerns.
