Patient Advocate

Every patient who enters the Premier Visiting Nurse program is provided free of charge a Patient Advocate. The role of the ‘Patient Care Advocate’ is to answer non-medical questions, locate resources, and meet the needs of the patient and/or their family. The patient advocate also coordinates with the Transitional Care Liaison to coordinate patient appointments, provides educational materials plus and other medical concerns.

The Patient Advocate provides needed support and eases concerns for the patient and his/her family at a critical time.
