Our continuum of care is broad and far-reaching. Some clients may be in need of all these services. Others may only need one. Together we will develop a care plan with your physician to ensure all needs are being properly addressed.

  • Skilled Nursing — skilled nurses assess and monitor the patient’s health care needs, educate patients and caregivers, and collaborate with attending physicians to promote continuity of care.
  • Physical Therapy — through therapeutic exercise, balance activities and ambulation training, our physical therapists help patients regain functional mobility and gross motor skills.
  • Speech Therapy — our Speech Language Pathologists help patients regain their ability to produce and understand speech as well as facilitate communication skills. They also help patients manage swallowing disorders.
  • Medical Social Workers — Medical Social Workers specialize in evaluating the social and emotional factors that affect the ill and disabled. They help patients and their families manage the stress that can be put on families and connect them with the necessary community resources available to them.
  • Home Health Aides (HHA) — HHAs assist patients in their completion of activities of daily living (ADLs) and personal care.
  • Registered Dietician — our dietician provides consultations on appropriate diet management and meal planning.
  • Occupational Therapy — helping patients return to their everyday activities and regain their skills or cognitive abilities is the role of occupational therapy. Helping clients have independent, productive, and satisfying lives is the goal.
