When someone receives a cancer diagnosis, has had a stroke or heart attack or other major medical condition their entire family may be in crisis. Our goal is to make the process easier for the patient and to educate the family and caregivers about the best way to help their loved one throughout the course of care. Our nursing staff navigates and/or coordinates patient appointments, provides educational materials, and provides support for the patient and his/ her family at a critical time. The Transitional Care Liaison also works with the patient advocate which every patient has when they become a patient with Premier. Whatever a patient’s needs might be, we are able to address the needs and questions through:

- Cancer Fatigue Management
- Palliative (Comfort) Care
